Job Description
This Clinical Social Worker position is part of Intermountain?s Mental Health Integration \(MHI\) program\. MHI is a team\-based care model that integrates mental and physical health into primary care practice \. MHI team\-based care is outcome\-oriented, and supports Intermountain?s Triple Aim for population health meaningit improves the quality of health,enhances the patient experience, andlowers the cost of care\. As the Mental Health Specialist within the MHI model, you will work directly with the Primary Care Provider and team toclarify complex patient diagnoses,plan effective treatment, and provide solution\-focused interventions \. In the Clinical Social Worker role, you will consult with the Primary Care Provider and care managers to support the prevention, treatment, and management of chronic disease for patients? and their families\.

Essential Job Duties
+ Assess the patient's medical history, mental health history, and current presentation to understand the patient's needs from treatment\.
+ Diagnose and treat various mental, emotional and behavioral disorders\.
+ Develop a plan of careeither individually or with input from the patient, patient family/significant others, physicians and other members of the healthcare team\.
+ Modify care plansto reflect changes in the patient's status\.
+ Refer patients to other servicesto improve their well\-being\. These referrals may include community services, treatment programs, support groups, psychiatrists, or other mental health providers\.
+ Properly documents pertinent information in regard to the patient's plan of care\.
+ Work with the multi\-disciplinary team todevelop a discharge planin order to provide further care, services, and referrals in order to ensure continuity of care\.
+ Comply with federal, state, local and clinical program/facility standards\.
+ Act within the scope of the Mental Health Practice Act\.

+ Shift Details: This is a full\-time exempt position\.
+ Additional Details: This position will work in Legacy OB/GYN in American Fork, American Fork Internal Medicine and the Highland Family Practice Clinic\.
Minimum Requirements
+ Master's Degree in Social Work from an accredited institution\. Degree will be verified\.
+ Current State Licensure as LCSW\.
+ Computer Skills, i\.e\., email, word processing\.
+ Excellent communication skills; verbal and written\.
Physical Requirements
+ Hearing/Listening, Manual Dexterity, Seeing, Sitting, Speaking, Walking, Standing\.
Preferred Requirements
+ Experience in an outpatient mental health setting\.
+ Experience working with multi\-disciplinary teams\.
+ Experience with family medicine and/or internal medicine practices\.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
